Elmwood Assisted Living & Skilled Nursing of Fremont

Elmwood Assisted Living & Skilled Nursing of Fremont is a single-floor Victorian style facility adorned with several beautiful turrets, shaker shingle roofing, and a grand front porch dressed with comfy wicker seating for those warm afternoons.  The Victorian theme continues within the grand foyer, library, ice cream parlor, solarium, and dining room.  Four beautiful neighborhoods within the community each house twelve suites, a parlor, a lounge area, as well as a fully equipped family kitchen and laundry area. This provides a nice area where residents can entertain family and friends, or attend to their own laundry, if desired. An exterior, paved walking path, dressed with benches, surrounds the community so our residents can enjoy daily walks or just sit and enjoy the beautiful grounds and wildlife.

Elmwood of Fremont offers 24-hour nursing and a secured neighborhood specifically devoted to the care of individuals with Alzheimer’s disease and other memory impairments.  A beautiful, secured garden is also provided to allow for safe outdoor activity, walks, and gardening.

Licensed by the Ohio Department of Health as a 48-suite Assisted Living / 18-suite Memory-Care and 30-suite Skilled Nursing and Long-Term Care facility. Elmwood Assisted Living of Fremont is a member of Assisted Living Federation of America, Ohio Assisted Living Association, Ohio Healthcare Association, and the Chamber of Commerce of Sandusky County.

Elmwood Assisted Living of Fremont is currently certified by the Ohio Department of Aging to provide basic assisted living service and memory care service and is currently accepting referrals for individuals who are enrolling in the Medicaid waiver program or My Care Ohio. SUITES

Elmwood of Fremont offers numerous suite floor plans. Each is equipped with a single-size bed and linens, a nightstand, beautiful floor-to-ceiling windows, and lovely draperies for your privacy.  Residents may bring their own favorite furnishing, pictures, trinkets, and items to personalize their new home which adds to their comfort and reflects their unique personality.

Two-Room Suite – offers a spacious floor plan which includes a bedroom, living room, kitchenette, and private bathroom.  It is equipped with generous shelving for personal items and a walk-in shower.  The living space offers large closets, beautiful floor-to-ceiling windows, providing natural lighting, and beautiful views.  This suite is a wonderful option for individuals, couples, or two individuals who wish to share it as a Companion Suite.

Turret Suites – have the added feature of a beautiful, windowed turret with a panoramic-view which provides additional living space, perfect for a living room area.  Residents will appreciate the generous closet space, a lovely kitchenette, a large, private bathroom with plenty of shelving for personal items, and a walk-in shower.

Private Studios – offer comfortable living space equipped with kitchenette, large living space, private bathroom with walk-in shower and generous closet space.  Several of these suites accommodate couples very comfortably.
